Job Description
The Iroquois Center is in search of a CASE MANAGER. This position is a collaborative process that is designed to assess, plan, and facilitate the needs of an individual or family's comprehensive health needs.
This full-time position would also provide attendant care, parent support, wraparound facilitation, and assistance with psychosocial groups as needed. This position also works with children/adolescents with serious emotional disturbances, with goals to teach them how to manage behaviors to reach their treatment plan objectives.
Experience in treatment planning, problem solving, crisis intervention, as well as working with multi-disciplinary agencies would be helpful.
Candidate must have excellent social and communication skills.
This position requires a bachelor’s degree and/or comparable work experience.
